New! - After downloading the form, you may either print it and fill it out by hand or fill it in using your computer and printing a finished form.  You cannot submit the form over the internet or save your data.  You must use the printed form as your copy and also the form that you will turn in at the office.  Once the form is downloaded, you may want to click the box in the upper right corner of ADOBE READER titled "Highlight Fields."  It is not necessary but the fields are easier to find.  You still must sign the form and if charging your fee, circle the card of choice.
